PACE North provides an all-inclusive care program for adults 55 and older in northern Michigan, delivering nursing-home-level care in participants’ homes. A capitation funding model from Medicare, Medicaid, and private pay supports coordinated services, while an Interdisciplinary Team builds individualized care plans that include medical care, medications, home care, therapy, transportation, meals, and social activities, often centered at a Day Center. Unlike fee-for-service models,PACE North offers a single, integrated approach across seven counties to manage medical, social, and functional needs in one program.
