Consumer Directed Choices and Healthy Alliance partner to expand New York State Medicaid 1115 Waiver services across the Capital Region, Central New York, and North Country. Consumer Directed Choices (CDChoices) is pleased to announce a new partnership with Healthy Alliance, the designated Social Care Network (SCN) Lead Entity for the Capital Region, Central New York, and North Country under the New York State (NYS) Medicaid 1115 Waiver Program. This partnership represents a significant milestone as CDChoices continues to expand access to community-based services that address the social factors affecting health and well-being. Through this partnership, CDChoices will deliver Screening, Navigation, and Enhanced Care Management services to Medicaid-eligible individuals throughout the region. These services are designed to identify and address Health-Related Social Needs (HRSNs), connecting individuals with the resources and support they need to improve their health, stability, and quality of life. The New York State Medicaid 1115 Waiver Program aims to strengthen connections between healthcare providers, community-based organizations, and social service agencies to better address non-medical factors that influence health outcomes. Through comprehensive screenings, CDChoices will identify unmet social needs such as food insecurity, housing instability, transportation barriers, and other challenges that may affect an individual's health and independence. Working with Healthy Alliance, CDChoices will connect eligible Medicaid members to Enhanced HRSN Services and provide navigation support to help individuals access state and local resources. In addition, CDChoices will also deliver Enhanced Care Management, offering a coordinated, community-based approach that addresses social needs, promotes better health outcomes, and encourages greater self-sufficiency. Healthy Alliance, a nonprofit organization dedicated to advancing health equity, serves as the SCN Lead Entity across the Capital Region, Central New York, and North Country. Through its extensive network of healthcare providers, community-based organizations, and social service partners, Healthy Alliance ensures Medicaid members can access the services and supports they need. By partnering with Healthy Alliance, CDChoices is expanding its ability to serve individuals in communities where it has strong roots and established relationships. Together, the organizations will work to strengthen the integration of health, behavioral health, and social care services while increasing access to vital community supports for Medicaid members throughout the region.
