NCFCU grant supports Adult Home Healthcare at CVHHH. Announcements Community News Press Releases Central Vermont Home Health & Hospice (CVHHH) has received a $7,200 grant from NorthCountry Federal Credit Union (NCFCU) to support the agency's Adult Home Healthcare program. This program provides intermittent, short-term medical care, education, and support at home to more than 1,700 Central Vermonters each year. Adult Home Healthcare is designed for patients who are recovering from a hospital stay, injury, or surgery, or who need help managing their chronic disease. Individualized care plans, developed in collaboration with a person's physician, help Central Vermonters regain their strength and independence, take a more active role in their care, and achieve the best-possible health outcomes and quality of life. Services are delivered in the comfort and privacy of home by an interdisciplinary team of experienced registered nurses, physical, speech, and occupational therapists, social workers, and licensed nursing assistants. CVHHH is grateful for NCFCU's ongoing championship of its work and mission. Medicare - its primary payor for home health care services - continues to cut reimbursement for home health services delivered, creating a disparity in the value of the care provided by CVHHH clinicians and what the agency is reimbursed each year. This funding gap makes support from community partners like NCFCU vital to sustaining Central Vermonters' access to Adult Home Healthcare and other CVHHH services.
