Jaan Health has secured over $25 million in funding to expand its AI-powered proactive care platform Phamily, which manages chronic disease patients between doctor visits. The round includes $15 million in non-dilutive growth capital from Level Structured Capital, an affiliate of Level Equity. Phamily currently serves over 150 healthcare organisations, supporting hundreds of thousands of patients with more than 250 different chronic condition diagnoses. The platform uses proprietary AI to enable healthcare providers to deliver high-quality care management at scale whilst addressing labour shortages and rising costs. Founded nearly a decade ago, Jaan Health built an eight-figure business without institutional capital before this funding round. The company plans to use the investment to expand into new market segments and penetrate health systems and integrated networks across the United States.

Evergreen Nephrology has partnered with Phamily, Jaan Health's AI-powered care management platform, to launch Evergreen Connected Care, a proactive kidney care management service. The collaboration combines Evergreen's nephrology expertise with Phamily's AI capabilities to improve care for patients with chronic kidney disease. Evergreen Connected Care enables nephrology practices to deliver population-wide care through AI-enhanced clinical decision support, comprehensive care planning, and omni-channel patient communication. The service aims to improve patient engagement whilst reducing administrative burden on nephrologists, allowing them to spend more time with patients. Operating across 24 states with over 1,000 providers, Evergreen Nephrology focuses on value-based kidney care models. Phamily has over a decade of experience in AI-based care management, with technology tested across millions of patient encounters.
