Responsibilities
  • Describes the PACE program, either by telephone, electronically, or in-person to interested persons including candidates for enrollment, caregivers, or others in the community interested in the PACE program.
  • Escalates unresolved issues to supervisor and/or manager. Collaborates with other teams to resolve issues.
  • Serves as a liaison between the hospitals, physicians, different clinics, as well as community agencies to ensure continuity and quality of program services.
  • Answers initial calls for PACE referrals and inquiries.
  • Referral tracking and follow up.
  • Collaborates with Intake Coordinator to schedule initial in person assessments.
  • Verify Medicare benefits.
  • Work with applicants on initial Medicaid application.
  • Work with participants on annual Medicaid redetermination.
  • Follow up with IDT regarding expired authorizations.
  • Assist with Enrollments into system as back up.
  • Notify IDT of schedule updates or changes immediately as they occur.

  • Associate's Degree- Preferred
  • 1 year Experience in a PACE program- Preferred
  • 1 year Experience with Medicaid- Preferred
  • 1 year Experience in a health-related setting- Preferred
