Requirements
  • Master's Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT)
  • One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations
  • Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW)
  • Employment eligibility verification
  • CPR/First Aid training
  • Tuberculosis Test
  • Background Screening
  • Drug Test
  • Reliable transportation
  • Valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties

Responsibilities
  • Perform initial comprehensive biopsychosocial assessment
  • Develop a plan of intervention
  • Collaborate with the client to develop an individualized treatment plan
  • Provide ongoing case management support
  • Provide field-based therapeutic supportive case management services
  • Consult with LCSW and supervisor to conduct crisis and suicide risk assessment
  • Provide crisis intervention services
  • Provide coverage to the 24/7 After Hours Response phone line
