Transitions Care provides specialized end-of-life care, including hospice, palliative care, and chronic-illness management. It operates a 24/7, patient-centered care model delivered by interdisciplinary teams (physicians, nurses, social workers, dietitians, chaplains, and volunteers) to create individualized care plans. Care is offered in homes, assisted living facilities, and hospitals, with additional services such as music, massage, pet therapy, and bereavement support. Compared with competitors, Transitions Care emphasizes a continuous, full-spectrum approach across care settings, strong personalized planning, and a focus on comfort, dignity, and emotional/spiritual well-being, supported by a combination of Medicare/Medicaid/private payer revenue and strategic growth through organic expansion and acquisitions. The company’s goal is to help patients experience their final life stages with comfort and dignity while supporting families through bereavement and ongoing care needs.
