Requirements
  • Bachelor’s degree or equivalent years of experience
  • Excellent interpersonal skills, organizational, administrative and communication skills
  • Knowledge of funding agency regulations/requirements
  • Must have demonstrated computer literacy
  • 18 years of age or older
  • Ability to provide on-call support
  • Must have satisfactory background checks
  • Valid driver’s license and safe driving record
Responsibilities
  • Creates positive relationships with all internal and external stakeholders including employees, peers, local and state agencies as well as parents and guardians
  • Ensures all communication is timely and occurs in a manner that is professional, clear and concise
  • Builds trust, mutual respect and cohesion around a shared mission
  • Provides ongoing support of the Home Care Providers including assisting with the selection of new Home Care Providers, ensuring active Home Care Providers are current with training and certifications and coordinating routine/preventative medical care for supported individuals
  • Routinely makes home visits, complying with regulatory guidance along with conducting necessary home study evaluations and recertifications of homes when required
  • Ensures adherence to organization-wide policies and procedures and state and federal regulations, meeting licensing and certification standards
  • Champions an environment in which supported individuals have choices, are treated with respect and dignity and actively engaged in their community
  • Provides ongoing developmental support of the Home Care Providers, ensuring they are meeting ongoing programmatic requirements
  • Coordinates services to provide medical, psychiatric and social-emotional needs of the supported individuals
  • Fosters an environment where the dignity, decisions and rights of the individuals are valued
  • Coordinates with and provides support to other departments such as Quality Assurance/Quality Enhancement, nursing, clinical and other administrative departments
  • Fosters a positive work environment while supporting a culture of teamwork, growth, collaboration and communication
  • Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity
  • Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role
  • Conduct training as required and assigned
  • Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation
  • Accept reassignment as required
