Requirements
  • A high school diploma and one year of experience in human services or a related field are required.
  • One year of administrative experience is required.
  • At least two years of experience working with individuals diagnosed with intellectual disability are required.
  • AMAP, SCIP-R, and First Aid/CPR certification is required.
  • A valid driver’s license is required, along with the ability to drive a large vehicle such as a wheelchair-accessible or 15-seat van.
  • The role requires respect and consideration for residents being served.
Responsibilities
  • Coordinate and implement in-house programming, including assessments, goal planning, daily progress evaluation, monthly resident reviews, recreational programming, community-resource development, and maintenance of the van trip log.
  • Ensure individuals receive required services, that day-to-day needs are met, and that services are delivered according to their individual plans.
  • Assess individuals’ and families’ holistic needs, make appropriate internal and external referrals, follow up on referrals, and participate in cross-program service reviews.
  • Organize records and ensure they remain current.
  • Participate in interdisciplinary team and staff meetings.
  • Create meeting agendas with the Program Manager, including committee reports, announcements, and staff concerns.
  • Collaborate with consultants, especially the psychologist, on programming.
  • Follow through on recommendations from the utilization review committee and attend utilization review meetings when delegated.
  • Complete and maintain investigation certification related to incidents within the developmental disabilities services program.
  • Ensure life-safety safeguards are implemented and followed.
  • Maintain and submit timely, accurate records and reports concerning individuals’ health and safety and program quality.
  • Submit, update, and close program incidents and maintain compliance with the Office for People With Developmental Disabilities and the Justice Center.
  • Make records available for IPR and utilization review.
  • Prepare Program Specialist staff schedules, manage vacation, personal, and sick leave, and maintain coverage during staff shortages or emergencies.
  • Ensure staffing patterns comply with facility policy.
  • Conduct scheduled supervisor meetings concerning programmatic and staffing issues.
  • Evaluate staff work performance through observation and performance counseling in conjunction with the Program Manager.
  • Ensure individuals receive or are assisted with skill development in activities of daily living, domestic living, recreation, and socialization.
  • Assist the Program Manager in developing and implementing agency and program policies in accordance with Office for People With Developmental Disabilities and funding-agency guidelines.
  • Conduct frequent rounds across all shifts and report whether safety, health, environmental, care, and programming requirements are being met.
  • Track medical appointments; communicate with the CMS, nurse, physicians, and managers; maintain medication inventory accuracy; order refills; complete medical referral forms; and complete monthly medical-book reviews.
  • Coordinate transportation for day programming, recreation, medical appointments, and special requirements.
  • Ensure transportation to and from programs, community-inclusion activities, medical appointments, home visits, and other activities.
  • Complete Staff Information Entry Forms when escorting individuals to medical appointments.
  • Ensure timely receipt and availability of individuals’ toiletry items, clothing, and personal effects.
  • Prepare day-program progress reports for assigned ITT reviews.
  • Distribute petty cash to staff or collect petty-cash receipts and submit them to the Program Manager.
  • Conduct monthly fire-evacuation drills with staff on all shifts.
  • Participate in admission and discharge reviews and make recommendations to the Program Manager.
  • Develop monthly recreation schedules in consultation with the Program Manager.
  • Implement and monitor in-house and community recreational activities, providing hands-on assistance as needed.
  • Direct care as needed.
  • Work the stated attendance schedule of 1:00 PM to 9:00 PM, Sunday through Thursday, and other times at the Program Manager’s discretion.

  • An Associate’s Degree is preferred.
