Requirements
  • Graduation from an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR Basic Life Support certification.
  • Meeting the practice requirements in the state of employment.
  • At least 12 months of experience as a Registered Nurse is required for assigned oversight duties.
  • Successful completion of three months of experience as a Registered Nurse in home peritoneal dialysis and/or hemodialysis is required for assigned oversight duties.
  • The role is entry level for Registered Nurses with less than two years of nephrology nursing experience.
  • Ability to provide direct patient care involving heavy lifting, moving equipment and patients, and assisting with ambulation.
  • Ability to stand for prolonged periods and bend over.
  • Ability to move, with assistance, machines and equipment weighing up to 200 pounds and lift chemical and water solutions weighing up to 30 pounds to a height of five feet.
  • Ability to participate in an on-call rotation, including nights, weekends, and holidays, as required by program needs.
  • Willingness and ability to travel to training sites, other facilities, or patient homes as required.
Responsibilities
  • Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation, for an assigned group of patients.
  • Assess and manage patients’ responses to home dialysis training and treatment therapy according to prescribed protocols, and communicate patient-related issues to the physician as needed.
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
  • Perform essential functions under the direction of the Supervisor, with guidance from the Educator or Preceptor, or in collaboration with another Registered Nurse.
  • Collect and analyze dialysis data for assigned patients, document it in the medical record, modify treatment plans as indicated, and notify the Supervisor or physician as needed.
  • Assess, collaborate on, and document patients’ and families’ basic learning needs to provide initial and ongoing education.
  • Direct and provide, in collaboration with patients, home care partners, and care staff, all aspects of safe and effective dialysis therapy for assigned patients.
  • Administer prescribed medications or medications authorized by approved algorithms, and document medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource for the health care team and participate in staff training and orientation of new staff as assigned.
  • Ensure correct laboratory collection, processing, and shipping procedures, and reschedule missed or insufficient collections.
  • Identify expected outcomes and document and update nursing assessments and plans of care in collaboration with the Interdisciplinary Team.
  • Ensure patient awareness of transplant and treatment modality options.
  • Assist with identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners.
  • Assess Home Dialysis candidates’ home environments and identify barriers, partner readiness, and family readiness and ability to perform home dialysis treatments.
  • Train Home Dialysis patients and Home Partners in the safe and effective operation and maintenance of Home Dialysis equipment and treatment supplies through the formal training program.
  • Complete Clinical Advancement Program requirements to advance.
  • Rotate coverage with other licensed home therapies staff to ensure reliable and adequate coverage.
  • Perform other duties assigned by the Supervisor.
