Requirements
  • Candidates must hold an active New York State Licensed Clinical Social Worker or Licensed Mental Health Counselor credential.
  • One year of post-Master's Degree experience is required.
  • Medical clearance is required for this member-facing role, including evidence of immunity to MMR, Hepatitis B, and Varicella, and a tuberculosis screen, unless an approved medical or religious accommodation applies.
Responsibilities
  • The Behavioral Health Specialist is available for care team referrals to triage member needs, provide short-term therapeutic interventions, address the needs of members presenting with serious mental illness or substance use disorder, manage behavioral health escalations, and help facilitate transitions of care as needed.
  • Provide behavioral health support to members in an in-person and virtual setting.
  • Develop member care plans when behavioral health interventions are needed.
  • Provide clinical education to members on preventative care topics, including behavioral health and mental illness.
  • Make appropriate referrals for medical support, social resources, behavioral health, and Substance Use Disorder to external and internal providers or the internal care team.
  • Collaborate with internal care teams to assess and determine the need for member escalation or placement in different programs, such as lower- or higher-intensity programs.
  • Maintain engagement with high-acuity members.
