Requirements
  • Graduate of an ACEN- or CCNE-accredited school of professional nursing.
  • Hold a current, full, active, unrestricted registered nurse registration.
  • Hold current Basic Life Support certification.
  • Have experience in care coordination and chronic disease management.
  • Hold a current, full, active, and unrestricted license or certification, as applicable, in California, or otherwise meet VA Qualification Standards for the position.
  • Successfully complete VA credentialing and privileging review and a background investigation cleared through the Office of Personnel Management via the VA Security and Investigations Center before starting work; PIV credentialing is required.
  • Complete required VA mandatory training and maintain current licensure or certification documentation annually.
Responsibilities
  • Provide professional nursing services consistent with licensure, certification, and the assigned functional statement.
  • Collaborate with PACT staff to develop and manage care management plans and care coordination for the assigned patient panel.
  • Participate in secure messaging, virtual care, shared medical appointments, and face-to-face visits as part of care management.
  • Identify needs for discipline-specific referrals and enter consultation requests as needed.
  • Enter orders in the VA electronic health record for tests per approved standardized registered nurse protocols or primary care provider orders.
  • Use evidence-based guidelines and nursing protocols to promote patient engagement, self-care, and wellness.
  • Maintain a completion rate above 90% for preventive screenings and document interactions in CPRS within 72 hours.
