Requirements
  • An active, unrestricted Registered Nurse license in the applicable state or states is required.
  • A minimum of three years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role is required, or an equivalent combination of education and experience.
  • State-specified certification may be required based on state law or contract.
Responsibilities
  • Manage the member's case within the scope of licensure and provide supervision and direction to non-Registered Nurse clinicians participating in the case.
  • Develop, monitor, evaluate, and revise member care plans to meet member needs and optimize health care across the care continuum.
  • Perform face-to-face clinical assessments to identify, evaluate, coordinate, and manage members' physical health, behavioral health, social service, and long-term services and supports needs.
  • Identify members at high risk for complications and coordinate care with the member and health care team.
  • Manage members with chronic illnesses, co-morbidities, and/or disabilities to support cost-effective and efficient utilization of health benefits.
  • Obtain thorough and accurate member histories to develop individualized care plans.
  • Establish short- and long-term goals with members, caregivers, families, natural supports, and physicians, and identify members who may benefit from alternative levels of care or waiver programs.
  • Develop the member's service care plan and ensure access to the required services.
  • Facilitate authorizations and referrals for services within the benefits structure or through permissible extra-contractual arrangements.
  • Interface with Medical Directors, Physician Advisors, and interdisciplinary teams on care-management treatment plans.
  • Assist with resolving provider, claims, or service issues.
  • Direct and/or supervise the work of LPN/LVN, LSW, LCSW, LMSW, and other licensed non-Registered Nurse professionals by assigning tasks, verifying and interpreting member information, conducting additional assessments, and reviewing and providing input on their performance.
Desired Qualifications
  • A Master of Arts or Master of Science degree in Health or Nursing is preferred.
  • Willingness to travel to the worksite and other locations as necessary.
