- A valid State of Hawaii driver's license is required.

- Demonstrated expertise in analysis and decision making is required.
- Effective written and verbal communication and time management skills are required.
- Familiarity with resources used to assess regulations and third-party reimbursement parameters is required.
- Working knowledge of Google's G Suite is required.
- Respond promptly to referral inquiries and coordinate with referral sources to identify potential patients early in recovery.
- Conduct preadmission screening and clinical assessments through on-site reviews, electronic health record reviews, or telephonic evaluations.
- Recommend treatment options based on clinical assessments, patient needs, caregiver situations, and financial resources.
- Facilitate referrals and admissions by providing necessary information and alternative placement suggestions for patients denied admission.
- Serve as a liaison among the rehabilitation team, admissions team, patients, families, and external sources to ensure cost-effective and outcome-focused plans.
- Coordinate transfer of patient care data and facilitate transitions between referring facilities and rehabilitation settings.
- Assist with precertification and payer negotiations, providing plans of care to secure approval for patient stays.
- Prepare patients and families for admission by explaining programs, offering tours, and addressing questions about rehabilitation services.
- Monitor and report admission trends, denial data, and referral source activity to improve service utilization.
- Build and maintain professional relationships with referral sources, identify new opportunities, and adapt to marketplace changes.
- Promote rehabilitation services and educate stakeholders about program benefits, services, and outcomes through professional representation and public relations.
- Participate in professional development, stay updated on rehabilitation advancements, and achieve annual educational goals.
- Use sound judgment and ensure patient safety.
- Follow organizational and departmental policies and guidelines, the code of conduct, and the employee handbook.
- Perform other duties as assigned.
- Certified Case Manager certification is preferred.
- Experience rehabilitating chronically or catastrophically ill or injured individuals is preferred.

REHAB Hospital of the Pacific launches new Stroke Center of Excellence. Tuesday, May 12, 2026 When a stroke impacts an individual and family, choosing where to recover is a critical decision. For over 70 years, REHAB Hospital of the Pacific (REHAB) has served as Hawaii and the Pacific's only rehabilitation hospital. This May, in recognition of National Stroke Awareness Month, REHAB officially launched its new Stroke Center of Excellence (Stroke COE), providing an expert interdisciplinary team and therapy tailored specifically to the complex challenges of stroke recovery. An average of 3,275 Hawaii residents experience a stroke each year, making it the number one cause of chronic disability in the state, according to the Hawaii Department of Health. REHAB's Stroke COE sets a new standard for patient experience and fills the gap for seamless transitions across the full continuum of care. Where a patient goes for stroke recovery care truly matters. While skilled nursing facilities play an important role in the broader healthcare system, studies consistently show that - especially in serious cases of stroke - survivors treated at a rehabilitation hospital like REHAB regain more physical function and are significantly more likely to return home.
