- Graduate from an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association, or be eligible for the National Registration Examination of the American Occupational Therapy Association, or have two years of Occupational Therapist experience and achieve a satisfactory proficiency examination grade conducted, approved, or sponsored by the United States Public Health Service, subject to the stated statutory exceptions.
- Hold current licensure as an Occupational Therapist in the State of Ohio.
- Have a minimum of two years of experience as an Occupational Therapist.
- Have two years of home care experience and one year of supervisory experience if supervising Certified Occupational Therapy Assistants.
- Hold a current CPR card.
- Have a current health certificate or physical examination and TB testing results.
- Hold a valid Ohio driver's license and have a good driving record.
- Perform initial evaluations and ongoing patient assessments in response to patients' ongoing needs and physician orders.
- Initiate written plans of care and re-evaluate and update them as necessary.
- Provide skilled occupational therapy modalities within the therapy scope of practice.
- Document skilled care and services in compliance with federal and state laws and regulations and Agency policies and procedures.
- Participate in case conferences concerning multidisciplinary team responsibilities, patient progress, and plans for continued care.
- Address and support patient and family cultural practices when they do not harm others or interfere with the planned medical therapy.
- Provide patient, family, and caregiver education using verbal and written communication techniques that account for cultural, ethnic, personal needs, and preferences.
- Supervise Certified Occupational Therapy Assistants in planned and delegated services, including scheduled patient record reviews, conferences, and home visits at least every 30 days unless state law requires more frequent visits.
- Plan appropriate discharges and provide information about community resources addressing ongoing patient needs.
- Apply knowledge of federal and state regulations and Agency policies and procedures regarding patient care.
- Communicate appropriately and promptly with physicians and other healthcare team members regarding patient status changes, findings, and needs.
- Notify the physician, Director of Nursing, and other healthcare team members of changes in patient condition and the need to modify the plan of care.
- Maintain current knowledge and clinical skills.
- Participate in the Agency's Quality Improvement program.
- Attend Agency meetings and required in-service sessions.
- Obtain the continuing education credits required to maintain re-licensure.
- Maintain confidentiality of patient and Agency information.
