Responsibilities
  • Conduct initial intake to collect demographic data and assess program eligibility
  • Conduct comprehensive risk assessments with each client, developing individual client goals
  • Provide necessary case management services which assist clients to achieve basic self-sufficiency benchmarks and to meet client goals
  • Conduct regular check ins
  • Empower clients while providing them the tools necessary to meet their goals
  • Provide clients and their sponsors with resources and training as necessary to ensure core services are completed
  • Make internal referrals as appropriate
  • Document participant services and activities thoroughly and in a timely manner in accordance with program and World Relief Sacramento requirements
  • Maintain accurate, timely, and compliant casefiles
  • Update and maintain client information in protected, online databases (Dynamics, Teams, etc.)
  • Achieve grant requirements and deliverables through timely, high-quality services
  • Participate in on-going training to improve service delivery and ensure compliance
  • Review and regularly present the status and progress of each case to supervisor and/or manager
  • Conduct outreach in the community to recruit clients
  • Collaborate successfully across World Relief Sacramento staff and departments, as well as with external service providers to ensure holistic case management
  • Attend meetings as instructed by supervisor
  • Other duties as assigned
