Memoir Health provides a behavioral healthcare platform that enables home-based care for behavioral health and substance use issues. It helps health systems remotely manage patients with tools like remote patient monitoring, scheduling, virtual care, and outcomes management, powered by a data analytics engine that highlights cost-saving opportunities and supports automated workflows and one-click referrals. The platform differentiates itself by automating care journeys and delivering integrated, outcomes-focused care within managed care and other healthcare networks, reducing administrative workload for clinicians. Its goal is to make care more proactive, accessible, and coordinated from patients’ homes, improving outcomes while lowering costs, and Memoir Health was later acquired by Lumeo Health in 2020.
