Requirements
  • Proficient in Microsoft Programs (Word, Excel, Access, PowerPoint), Zoom, and web-based scheduling software
  • Proficient in written and oral communication
  • Strong leadership abilities, and ability to work independently and in a team
  • Must be able to work a flexible schedule at multiple sites including evenings, weekends, and holidays
  • Must be able to lift up to 50 pounds
  • Must be able to pass a Pre-Employment Physical Exam
  • Must be able to pass post-offer Tuberculosis clearances
  • Maintain and execute confidential information according to HIPAA standards
  • Ability to obtain and maintain CPR/First Aid Certification
  • This position does not require regular driving but may be needed occasionally
  • Experience in dealing with substance abuse, mental health, and crisis intervention is desired
  • Knowledge of Domestic Violence, Harm Reduction, Trauma Informed Care and Housing First Models is referenced as knowledge for position
  • Ability to make crisis interventions and provide advocacy and case management is required (from Experience section)
  • Bilingual Spanish/English is a plus (not required)
  • Experience in dealing with substance abuse, mental health, and crisis intervention desired (from Experience)
Responsibilities
  • Coordinate with and support participant’s assigned Housing Navigator/Case Manager to achieve participant’s housing goals, acting as a liaison between Interim Housing and Housing Navigation
  • Provide conflict resolution and crisis intervention by encouraging positive methods for problem solving and using restorative justice practices
  • Provide accurate and thorough communication and documentation about all incidents involving participants and complete necessary incident reports
  • Collaborate with Mental Health Specialists and co-located staff on site
  • Remain knowledgeable on best practices and interventions related to Domestic Violence, Harm Reduction, Trauma Informed Care and Housing First Models
  • Check participant items for contraband at entry to the site as necessary
  • Complete intake with participant upon entry into site, as needed
  • Complete and submit daily occupancy reports as necessary and required per contract
  • Assist with identified housing plan tasks as necessary
  • Answer incoming calls, emails and respond to program and participant inquiries
  • Perform daily room checks and support to facilitate room changes
  • Light cleaning duties to ensure that the site is presentable and habitable
  • Complete daily documentation, maintain case notes and service entries in HMIS, and complete occupant reports as needed
  • Maintain daily provider sign-in sheets and occupancy standards
  • Coordinate cleaning/laundry schedule for participants
  • Sort and distribute participant mail
  • Participate in site and community meetings, attend monthly program committee meetings, and training opportunities
  • Contact emergency services in collaboration/consultation with supervisor when necessary
  • Conduct hourly walk around grounds to ensure safety
  • Submit maintenance requests and supply orders, as needed
  • Assist in facilitating volunteer activities/events, as applicable
  • Schedule flexibility (day, swing, overnight) and site location flexibility (multiple sites)
  • Additional tasks, projects, and responsibilities as assigned by supervisor
