Requirements
  • Knowledge of organizational practices, policies, procedures, applicable laws, regulations, requirements, safety policies, outreach services, funding agencies, community health care, and vocational services.
  • Skill in operating word-processing, spreadsheet, and database software programs.
  • Skill in gathering, analyzing, and organizing information.
  • Ability to plan, implement, and evaluate individual client care programs.
  • Ability to drive safely and efficiently, including holding a valid New Mexico driver's license, liability insurance, agency insurability, a dependable vehicle, and a clean driving record.
  • Ability to travel throughout the Albuquerque metropolitan area and surrounding areas of New Mexico.
  • Ability to read, write, and understand English.
  • Ability to carry out verbal and written instructions.
  • Ability to maintain working relationships with individuals from varying social and cultural backgrounds and with people with disabilities.
  • Ability to work extended hours and varied work schedules.

Responsibilities
  • Contact participants and provide case management services to an assigned caseload according to program guidelines and scope of work.
  • Travel as required based on caseload and scope of work.
  • Orient participants to organizational policies and procedures, rights and responsibilities, mission, programs and services, grievance procedures, and confidentiality policies.
  • Provide case management and refer participants to support services such as medical evaluation and treatment and social services.
  • Build and maintain collaborative relationships with community agencies.
  • Maintain accurate and current case records and document participant contacts in a timely manner.
  • Complete required case notes, program documentation, and reporting within designated timeframes.
  • Review and submit participant budgets, reimbursements, and other required program documentation.
  • Complete, verify, and submit monthly billing within established timeframes.
  • Close inactive cases monthly to maintain an accurate caseload.
  • Attend staff meetings, provide updates on participants with complex issues, and receive new referrals.
  • Enhance professional growth through educational programs, current literature, in-service sessions, meetings, and workshops.
  • Ensure compliance with HIPAA.
  • Maintain the safety of persons served under the employee's supervision.
  • Implement CARF standards and follow the applicable code of conduct and business ethics policy.
  • Maintain confidentiality of privileged information.
  • Perform other related duties as assigned.
Desired Qualifications
  • A Bachelor's Degree in social services or a related human services field is preferred.
  • One to three years of work experience in case management or social work is preferred.
  • Certified Case Manager, Certified Rehabilitation Counselor, or Master of Social Work credentials are preferred.
  • Bilingual ability is preferred.
