Requirements
  • High school diploma or equivalent.
  • Graduate of an accredited school of nursing.
  • Current full and unrestricted Registered Nursing license in Alaska or another United States state.
  • Direct hire candidates must apply for an Alaska nursing license before the start date.
  • Agency staff must possess an active Alaska nursing license.
  • Basic Life Support certification from the American Heart Association.
  • At least one year of general nursing experience with at least six months of employment as a clinical nurse at a SouthEast Alaska Regional Health Consortium outpatient clinic, or at least two years of general nursing experience with a minimum of one year in an ambulatory setting or one year of case management experience.
  • Knowledge of SouthEast Alaska Regional Health Consortium services and other tribal health organizations in Alaska.
  • Knowledge of the nursing process and the ability to apply it in a clinical environment.
  • Knowledge of safety and infection control principles.
  • Proficiency in using computers and medical equipment.
  • Ability to lift or move up to 50 pounds.
  • Ability to stand for long periods, bend, stoop, climb, and move to the floor.
  • Ability to complete a Clinical Competency Assessment within three months of hire.
Responsibilities
  • Coordinate the delivery of appropriate and timely care for patients to promote quality and cost-effective healthcare outcomes.
  • Collaborate with medical providers and interdisciplinary team members to establish individualized medical goals.
  • Coordinate multi-disciplinary care conferences for high-risk or complex patients referred to case management.
  • Manage panel acute and chronic care needs and health maintenance by reviewing Electronic Health Record reminders and providing patient outreach.
  • Participate in case management reviews with a multi-disciplinary team of providers for high-need patients.
  • Advocate for patients and families to foster decision-making, independence, and self-care.
  • Facilitate patient referrals within the organization and to outside facilities, including tracking progress and providing status reports to providers.
  • Perform staff nursing duties including patient assessment, documentation, education, and administration of medications or therapies.
  • Maintain annual competencies including safety and infection control requirements.
  • Participate in the triage role and respond to emergency situations as a Registered Nurse.
  • Facilitate specialty clinic referrals by managing waitlists, reviewing referrals for appropriateness, and scheduling patients based on priority.
  • Prepare clinical spaces for specialty clinics and assist providers during medical procedures such as colposcopy and cystoscopy.
  • Act as the primary case manager for obstetric patients, managing intake, tracking, education, and outreach for postpartum care and newborn immunizations.
  • Communicate and coordinate video teleconferencing visits with specialty providers from the Alaska Native Medical Center.
  • Manage referrals and coordinate appointment times for Exercise Treadmill Tests.
Desired Qualifications
  • Bachelor of Science degree in Nursing.
  • National Case Management certification.
  • Two or more years of recent case management experience in any setting.
