- Graduate of an accredited practical nurse or vocational nursing program
- Three years nursing experience
- Community/home health or medical/surgical experience preferred
- Currently licensed as a LPN/LVN in the State
- Complies with accepted professional standards and practice
- Good verbal and written communication, and organization skills
- Must be a licensed driver with an insured automobile in good working order
- Possess and maintain current CPR Certification
- Understands philosophy of Hospice concept
- Understands needs of the terminally ill
- Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements and an operational vehicle
- Providing direct patient care under the supervision of a registered nurse
- Following the plan of care
- Providing treatments
- Working collaboratively with the members of the team to help meet positive patient care outcomes
