About our Team:
We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.
About the Role:
In this role, you will be a part of a cohort of staff providing direct services to members, who face economic and social challenges in addition to having mental and physical health needs, through our innovative care model. You will be working within our “hub”, a primary care practice that in many respects will look more like a community center than a doctor’s office.
As an RN in our Hub, you will work with the team by utilizing your nursing skills and as a clinical support for our Medical Assistants. In addition, you will manage a panel of members to improve their health holistically. You will work alongside, and will be supported by, a care team associated with each member, whose work is focused around a Member Action Plan (MAP). You will engage members in the hub, facilitating their on-site care and being a bridge between on-site care and field-based care.
- Act as the practice-based nurse, supporting on-site providers in facilitating an efficient practice that promotes member engagement, trust, and support
- Provide care team support from a nursing perspective, and act as a bridge between on-site care and field-based care
- Coordinate services and relevant Member Action Plan (MAP) goals and tasks both internally and with external providers
- Review clinical competencies with MAs
- Assist in rollout and training of new POCT or procedures
- Assist in population health management for assigned hubs
- Serve as primary vaccine coordinator for VFC program
- Oversee pediatric vaccine schedule and administration
- Provide episodic care management for identified members
- Serve as liaison with outside lab vendor
- Collaborate and coordinate member care with specialists
- Meet with members in the hub as extenders of the primary care providers and other care team members, including medication reconciliation, standing orders, minor procedures, DME coordination, imaging management, prior authorization, and disease management education and oversight
- Providing in person/telephonic triage for members
- Oversee lab compliance
- Follow-up with members as needed
- Go above and beyond to connect with patients and partners in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day
- Work daily within our custom-built care facilitation platform, to collect data, organize information, track tasks, and communicate with staff, patients, and family.
Requirements for the Role:
- Active Registered Nurse license in the state of North Carolina
