Requirements
  • An appropriate combination of education, training, coursework, and experience may qualify an applicant to demonstrate the required knowledge, skills, and abilities.
  • A Master's degree in counseling, Social Work, or a related field is required.
  • An active and unrestricted Licensed Professional Counselor or Licensed Clinical Social Worker license in Pennsylvania is required.
  • A minimum of 3 years of clinical experience in behavioral health is required.
  • Knowledge of behavioral health screening tools and evidence-based therapeutic interventions is required.
  • Strong telehealth proficiency and the ability to adapt to new technologies are required.
  • Excellent communication and collaboration skills are required.
  • The ability to work independently and within a team setting is required.
  • The ability to maintain clinical competency and comply with all applicable regulations is required.
  • The ability to travel up to 50% is required.
Responsibilities
  • Provide direct counseling and therapeutic interventions to patients using evidence-based approaches.
  • Develop personalized care plans addressing psychological and emotional challenges.
  • Collaborate with primary care providers, psychiatrists, and other healthcare team members to integrate behavioral health services into patient care.
  • Act as a resource for medical staff, ensuring alignment with evidence-based behavioral health practices.
  • Deliver telehealth services to patients in virtual settings.
  • Use telehealth technology to expand access to care and reduce barriers for patients.
  • Administer, score, and interpret validated behavioral health screening tools, including PHQ-9 and GAD-7, to identify patient needs.
  • Document screening outcomes and provide feedback to patients and the care team.
  • Develop action plans based on screening results and ensure follow-ups.
  • Act as a patient advocate to ensure timely access to appropriate behavioral health services.
  • Educate patients and families about behavioral health conditions, coping strategies, treatment plans, and available resources.
  • Provide case management services to ensure continuity of care, including referrals to community resources and specialized services.
  • Monitor and document patient progress, modifying care plans as needed.
  • Assist in developing and implementing behavioral health programs to meet patient and community needs.
  • Monitor program outcomes and recommend improvements.
  • Provide training to healthcare staff on behavioral health topics, tools, and best practices.
  • Serve as a resource for patients, families, and providers regarding behavioral health care.
