Requirements
  • A Bachelor’s level degree in Counseling, Social Work, Psychology or related field is preferred; substitution of work experience for degree may be considered on an individual basis.
  • Must have access to a car, a valid driver’s license, and current auto registration; must pass pre-employment Motor Vehicle Record check and annual MVR check; must provide proof of auto insurance and carry minimum liability coverage; depending on job descriptions transporting clients, additional liability coverage may be required.
  • Agency Affiliated Counselor license in the State of Washington.
  • Certified Peer Support Certification in Washington State.
  • Must complete an annual clinical competency test as a case manager.
  • Education may include a preference for Bachelor’s level degree but substitution allowed (stated in Qualifications).
  • Must pass a TB test at time of hire and annually; must obtain Basic Life Support CPR within 90 days of hire and maintain it.
  • Pre-hire and annual employee health screening required; annual influenza vaccine required (with medical or religious exemption possible, otherwise mask during flu season).
  • Computer proficiency in Word and Excel.
  • Must self-identify as a current or past consumer of mental health services; must have self-knowledge to manage mental illness and be well grounded in recovery for at least two years.
  • Must have understanding of homelessness and general knowledge of major classifications of mental disorders; strong commitment to consumer rights and community integration; ability to outreach to landlords and provide housing support.
Responsibilities
  • Participates in outreach services to prospective clients; coordinates outreach with outside agencies; collaborates with co-workers and personnel from other agencies.
  • Provide peer counseling and support, drawing on common experiences as a peer to validate client experiences and guide recovery.
  • Serve as a mentor to clients to promote hope and empowerment.
  • Participates in ongoing psychosocial assessment of psychological, social, vocational, systemic and economic goals and needs of the consumer.
  • Assess the client’s ability to obtain and maintain housing, identify problem behaviors, and develop interventions to reduce or eliminate those behaviors.
  • Develop an ongoing housing rehabilitation plan to help each client establish skills to find and maintain housing.
  • Monitor, compile and submit data, reporting, and paperwork for the PATH program; assist clients in obtaining housing; housing case management experience is a plus.
  • Provide individual supportive case management to help clients cope with symptoms of mental illness affecting housing.
  • Organize, design and develop program structure in consultation with the Manager of Housing & Homeless Services.
  • Develop or review manuals pertaining to program policy, procedures, and operating instructions in consultation with the Manager of Housing & Homeless Services.
  • Develop specific procedures and policies necessary for operation and implementation of newly designed or modified programs in consultation with the Manager of Housing & Homeless Services.
  • Provide interventions and/or referrals to other providers to assist consumers in meeting treatment and level of care needs; interventions may include individual, family, and group modalities, counseling, case management, vocational rehabilitation, system coordination, consumer advocacy, and crisis management.
  • Ensure treatment planning and interventions are client-centered, culturally competent and age appropriate; consider psychosocial, economic and cultural needs of consumer, family, and community; maintain confidentiality, safety, and self-determination.
  • Utilize community and natural supports to supplement services to better integrate the consumer into their community and graduate them when clinically indicated.
  • Ensure interventions are effective, appropriate, and timely by maintaining knowledge of psychiatric treatment, rehabilitation, community resources, laws, and agency policies.
  • Ensure program billable hour expectations are met; complete documentation accurately, timely; participate in tracking systems and chart audits; seek supervisory assistance for documentation issues.
  • Independently identify program needs and contribute to program development through meetings, committees, and independent projects to improve services.
  • Maintain safety, security, and confidentiality of consumers and families.
