Job Description
Under general supervision and according to established policies and procedures, provides assistance to Care Management functions for the site. Works under the direction of the RN and/or Social Worker to complete referrals for post-acute services for patients and assist with tasks that do not require a clinical license or degree. Responsibilities include the following activities\:
a) Arrange post-discharge services,
b) Create and follow up on electronic referrals using the Tenet Case Management system,
c) Review patient choice letters with patients/families for required signatures,
d) Provide follow up Important Message to Medicare patients prior to discharge,
e) Communicate with patients, families and other members of the Care Team,
f) Complete tasks assigned by Case Manager and Social Work staff,
g) Make copies, send faxes and complete phone calls,
h) Complete process reviews or audits as requested and
I) Other duties as assigned.

2. Preferred\: Associate or Bachelor’s Degree
3. Preferred\: Paramedic, EMT or Nursing Assistant Certification
4. Preferred\: Acute hospital experience
5. Preferred\: Working knowledge of medical terminology
Skills Required
1. While performing the duties of this job, the employee is regularly required to sit, talk and hear.
2. Excellent organizational skills
3. Excellent verbal and written communication skills
4. Demonstrated problem solving skills
5. Knowledge of managed care and referral processes
6. Routinely required to utilize fine motor skills (typing and data entry)
7. Ability to utilize computer systems; computer skills and knowledge of various software packages
8. Regularly push, pull and/or lift objects weighing up to 20 lbs; occasionally lift and/or pull up to 50 lbs.
9. Frequent requirements to stand, kneel, stoop and/or crawl.
10. Ability to understand and apply various third party payer requirements, verification procedures and benefits.
11. Ability to work within a complex environment and handle multiple priorities.
