Requirements
  • A Pharm.D. or Bachelor of Pharmacy degree from an accredited school of pharmacy is required.
  • A Basic Life Support Healthcare Provider certification is required within 90 days of employment; other resuscitation certifications may be required depending on the department.
  • A pharmacist license is required in the applicable state, including Illinois, Missouri, Oklahoma, or Wisconsin.
  • No prior experience is required.
  • The role requires frequent lifting, carrying, pushing, pulling, sitting, standing, walking, reaching, repetitive foot/leg and hand/arm movements, visual and depth perception, hearing and speech for oral communication, and keyboard use/data entry.
  • The role requires occasional bending, stooping, kneeling, squatting, twisting, gripping, and handling objects weighing 25–50 pounds, as well as rare climbing.
Responsibilities
  • Ensure the safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, laboratory value monitoring, and dose or therapy adjustment as applicable.
  • Document medication-related plans and interventions in the electronic medical record.
  • Provide clinical consultation with complete, evidence-based information to practitioners.
  • Recommend and encourage safe and cost-effective pharmacotherapy for patients through multidisciplinary collaboration.
  • Oversee the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy.
  • Supervise pharmacy technicians and interns who support the pharmacist in the drug delivery system.
  • Precept pharmacy students and residents as applicable.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
