Requirements
  • Must hold, at minimum, a Licensed Practical Nurse certification acknowledged by the State of Arizona.
  • Must have at least three years of experience in an assisted living or long-term care environment.
Responsibilities
  • Prepare and update resident service plans as required.
  • Perform assessments on prospective residents to determine the appropriate level of care for assisted living or memory care.
  • Oversee each resident’s medication delivery plan, including self-administration and medication monitoring, medication ordering, physician orders, and reconciliation of medication administration records with physician orders.
  • Perform weekly narcotic counts and maintain documentation of narcotics provided to residents.
  • Schedule weekly showers and review caregiver documentation of activities of daily living.
  • Maintain resident injections, including tuberculosis skin testing, pneumonia vaccinations, influenza vaccinations, and vitamin B-12 injections.
  • Assist the Assisted Living Manager with implementing and coordinating policies and procedures and Arizona Department of Health Services rules and regulations for the assisted living and Memory Support floor.
  • Assist the Assisted Living Manager with reviewing and revising policies and procedures governing nursing and caregiving services in the assisted living facility.
  • Assist with developing and using departmental policies and procedures and establish effective communication between departments.
  • Interpret facility policies and procedures for employees, residents, family members, visitors, and government agencies as necessary.
  • Assist the Assisted Living Manager with planning, conducting, and scheduling caregiver in-service training, on-the-job training, and orientation programs.
  • Maintain a warm, calm, and positive environment throughout the facility.
  • Attend workshops and seminars to remain current in the long-term care field and maintain professional licensure.
  • Assist the Assisted Living Manager in maintaining a clean and safe facility and ensuring that necessary equipment and supplies are available.
  • Ensure that facility personnel, residents, and visitors follow safety regulations, including fire protection and prevention, smoking regulations, and infection control.
  • Maintain the confidentiality of resident information.
  • Ensure residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property, civil rights, and wage complaints are maintained.
  • Conduct routine facility inspections to ensure established policies and procedures are implemented and followed.
  • Maintain effective working relationships with medical professionals and other health-related facilities.
  • Review accident and incident reports and participate in the campus risk management committee.
  • Assist the Assisted Living Manager and Assisted Living Department with maintaining compliance with Arizona state rules and regulations.
  • Assist with implementing and maintaining the Comfort Matters programs.
  • Assist the Assisted Living Manager with risk and quality management programs.
  • Follow up on residents’ conditions and caregivers’ concerns.
  • Assist the assisted living team with the health and wellness of residents in Oasis and M2.
  • Answer emergency calls from residents’ team members and families after hours and on weekends.
