- Graduate of an accredited School of Vocational/Practical Nursing.
- Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC.
- Must have a current driver's license.
- Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting.
- Home health experience highly preferred.
- HCHB EMR experience highly preferred.
- Proficiency in nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Reliable transportation and valid, current auto liability insurance.
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report any changes to the supervising RN.
- Administer medications and treatments as prescribed.
- Educate patients and their families on health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
Jet Health provides home-based medical and hospice care in midsize markets. Clinicians visit patients at home to deliver services for conditions such as congestive heart failure, orthopedic needs, and infusion nursing, including care for catastrophically injured individuals, while coordinating with families and managing treatment plans. Growth comes from organic expansion, acquisitions of other home health and hospice providers, and building new operations, along with partnerships with referral sources, payors, and providers. Its goal is to deliver high-quality, comfortable care at home and to expand its market reach and service offerings through a network of home health and hospice services.
