Requirements
  • A bachelor's degree or higher from an accredited school of Nursing.
  • Five years of hospital clinical nursing experience, including two years in case management.
  • Texas Registered Nurse licensure through the Texas Board of Nursing or compact licensure, with a permanent Texas license required within 60 days when establishing Texas residency.
  • A Magnet ANCC-recognized case management certification: ACHPN-HPCC, CCM, CMC, ACM-NBCM, CDCES, CHPN-HPCC, CMGT-BC, CM-ABOHN, CMCN, or ANCC-NCM.
  • Sufficient proficiency in speaking, reading, and writing English to perform essential job functions, particularly those affecting patient or employee safety or security.
  • Comprehensive knowledge of Medicare, Medicaid, managed care requirements, community resources, healthcare financial and payer requirements, eligibility for state, local, and federal programs, discharge planning, utilization management, case management, performance improvement, managed care reimbursement, pre-acute and post-acute care venues, and post-acute community resources.
  • Strong assessment, organizational, and problem-solving skills, including the ability to prioritize multiple tasks and role components.
  • Critical-thinking ability and evidence-based analytical decision-making in interactions with physicians, payers, patients, and families.
  • Computer proficiency with the entire Microsoft Office Suite, including Access, Excel, Outlook, PowerPoint, and Word.
  • Ability to work independently.
Responsibilities
  • Collaborate with physicians and the multidisciplinary healthcare team to facilitate care for designated assignments and monitor medical progress, intervening as needed to ensure patient-focused, high-quality, efficient, and cost-effective care.
  • Communicate with physicians, the multidisciplinary care team, patients, families, and payers to ensure safe and timely transitions of care, including facilitating payer conversations to expedite authorization for appropriate post-acute placement.
  • Identify discharge barriers; plan, anticipate, prevent, and manage emergent situations; facilitate assessment and intervention to reduce readmissions; modify care and discharge plans as clinical conditions change; assign appropriate levels of care; complete required documentation; and eliminate discharge barriers.
  • Advise and support care-team members in managing complex psychosocial or medical conditions and formulate discharge plans to reduce avoidable delays.
  • Ensure compliance with Centers for Medicare & Medicaid Services Conditions of Participation and other regulations affecting case-management practice.
  • Facilitate referrals and transfers for home health care, hospice, durable medical equipment, and post-acute services, incorporating social determinants of health into decision-making.
  • Conduct comprehensive chart reviews for appropriate level of care, medical necessity, continued hospitalization, and discharge readiness; document assessments and interventions; and take action to expedite care and facilitate discharge.
  • Conduct post-discharge inpatient-record reviews for compliance with quality indicators and intervene to support compliance with Centers for Medicare & Medicaid Services guidelines and other performance measures.
  • Provide compliance reports, analyses of problematic areas, and improvement actions to appropriate parties.
  • Perform peer-to-peer evaluations, track and trend noncompliance issues, formulate Plan-Do-Check-Act activities, and report quality-of-care concerns to the appropriate governing body.
  • Identify opportunities to improve patient satisfaction in discharge and care-coordination domains and collaborate with department leadership on evidence-based patient-engagement strategies.
  • Manage length of stay for assigned cases, identify trends affecting length of stay, and resolve problems impeding treatment progress.
  • Educate and support timely discharges for uninsured patients and hospitalized outpatients while informing physicians of disposition options that maximize patient benefits and hospital reimbursement.
  • Partner with case-management leadership on length-of-stay management and medical appropriateness for resource utilization.
  • Identify opportunities for practice changes and independently implement innovative solutions through evidence-based practice, performance-improvement projects, and shared-governance activities.
  • Serve as a preceptor for case managers and social workers; implement staff education; coach and mentor staff and nursing students; serve as an expert case-management resource; and educate physicians, nurses, and other healthcare providers on case-management topics.
  • Identify opportunities for practice changes and implement evidence-based improvement solutions in collaboration with management.
  • Complete ongoing professional development activities and maintain the My Development Plan.
  • Be available for on-call work during emergencies.
  • Travel within the Houston metropolitan area as required.
