Responsibilities
  • Ensure families receive timely, quality services.
  • Serve as a liaison between individuals and families, community support professionals, and Care Managers as needed.
  • Communicate regularly with individuals receiving services and/or families regarding services provided.
  • Support the development of Circles of Support.
  • Attend individuals' Life Plan and Circle of Support meetings as needed and as outlined by OPWDD guidance documentation.
  • Develop Staff Action Plans with individuals, families, Care Managers, and community support professionals as needed, ensuring plans are accurately written and implemented.
  • Monitor service documentation, including monthly summaries, plans, reports, required documents, and distribution documentation.
  • Maintain required documentation in eVero in compliance with service ADMs and agency procedures.
  • Assess individual and family staffing needs; recruit, hire, train, credential, supervise, and coordinate backup staffing; and initiate service referrals as needed.
  • Support families with staff performance evaluations and Core Competencies.
  • Maintain active lists of staff.
  • Maintain confidential files for participating individuals and supervised staff.
  • Assume leadership for assigned program areas, including areas of the Community Support Services Department.
  • Assist in organizing trainings and conferences related to Community Support Services and staff development.
  • Promote public awareness of Racker's Community Support Services through ongoing community contact.
  • Provide leadership and direction to staff that fosters independence and positive choices.
  • Facilitate and support inclusion and community relationships for service participants.
