- Conduct patient assessments and complete skilled-visit documentation, including OASIS.
- Deliver high-acuity home health care focused on wound care and infusion therapy.
- Develop and maintain individualized plans of care through ongoing communication with physicians, patients, families, and interdisciplinary team members.
- Advocate for patients and ensure continuity of care across all disciplines.
- Manage patient cases through care coordination, timely follow-up, and proactive barrier resolution.
- Communicate changes in patient condition, obtain and track physician orders, and coordinate services with physical therapy, occupational therapy, speech therapy, medical social work, and home health aide providers.
- Administer medications and treatments as ordered while educating patients and caregivers.
- Collaborate with internal clinical and operational teams to ensure safe, timely, and reliable care delivery.
- Supervise and provide guidance to licensed vocational nurses and home health aides as appropriate.
- Participate in case conferences and clinical in-services focused on quality, safety, and patient outcomes.
- Experience with Homecare Homebase or a similar electronic medical record.
- Experience coordinating care for medically complex patients.
- Emergency department, intensive care unit, or other acute-care nursing experience.
