- Maintain accurate and confidential records of collected data.
- Prepare the Hospice CAHPS mailing list monthly before submission to Press Ganey.
- Calculate response rates for surveys and other data collection processes.
- Coordinate the timely and accurate implementation of telephone surveys and collate responses.
- Enter survey and Quality Assurance and Performance Improvement data into the appropriate computer program.
- Prepare mailings and meeting arrangements and take minutes for Calvary @Home Quality Assurance and Improvement, Professional Advisory Committee, and Performance Improvement meetings.
- Maintain accurate education records for each employee within Calvary @Home.
- Collect Medicare D information and notify the appropriate Medicare D plan upon admission and discharge for Medicare hospice patients.
