Requirements
  • An active, unrestricted compact-state Registered Nurse license is required.
  • Two to five years of bedside nursing experience is required.
  • The ability to collaborate via telephonic interaction with clinical and non-clinical team members to facilitate care coordination outcomes is required.
  • Proficiency in motivational interviewing techniques is required.
  • Proficiency in verbal and written communication is required.
  • Analytical skills are required.
  • Proficiency in word processing, spreadsheet, and email applications is required.
Responsibilities
  • Promote quality patient care, ongoing patient support, and case management for the renal patient population through guidance, support, care coordination, education, and other program-defined case management functions.
  • Manage implementation of assigned programs while serving as a subject matter expert and resource for patients, dialysis staff, physicians, and other healthcare professionals.
  • Develop and facilitate coordination of care for value-based program patients through telephonic support using nursing knowledge, data analysis, medical-information review, collaboration, and knowledge of clinical processes.
  • Lead assessment of patient conditions with clinic staff, identify unreported medical or social conditions and changes at home, and refer concerns to appropriate sources.
  • Document interactions and health information accurately, maintain patient tracking logs, and generate and analyze reports to identify trends, anomalies, and concerns.
  • Identify, review, assess, and allocate patients for program participation according to their needs and applicable policies and clinical guidelines.
  • Collaborate with the medical office, clinical staff, physicians, and other healthcare professionals to develop individualized and comprehensive plans of care.
  • Lead and ensure coordination and integration of services, revising care plans as needed to support continuity and cost-effective delivery.
  • Use motivational interviewing techniques to influence members to engage in their care.
  • Participate in process improvement, quality improvement, data collection, auditing, and project-based activities.
  • Maintain current nursing knowledge and best-practice guidelines, including chronic kidney disease and end-stage renal disease.
  • Evaluate program processes and recommend improvements to management.
  • Operate within the state's Registered Nurse scope of practice and maintain the integrity of protected health information.
  • Collaborate with providers and dialysis clinics to identify and mitigate hospitalization and mortality risk.
  • Escalate issues to a supervisor or manager when necessary and refer higher-level problems to senior staff.
  • Monitor newly enrolled patients and ensure appropriate onboarding to the GLP-1 program.
  • Track medication titration progress and identify patients requiring clinical review or intervention.
  • Monitor medication adherence, identify barriers, and engage patients at risk of discontinuation.
  • Track patient attrition, identify contributing factors, and support clinically appropriate re-engagement.
  • Provide ongoing monitoring and outreach for existing GLP-1 patients.
  • Collaborate with clinics, nephrologists, and the broader clinical team to identify patients requiring assessment or intervention and support escalation.
  • Facilitate referrals and communication with other providers to support coordinated treatment, information sharing, and continuity across care settings.
  • Monitor implementation of the GLP-1 algorithm, identify issues or exceptions, and help ensure consistent application.
Desired Qualifications
  • A bachelor's degree is preferred.
  • Certified Case Manager certification is preferred.
  • Certification in a nephrology specialty, such as Certified Nephrology Nurse or Certified Dialysis Nurse, is a plus.
  • Dialysis nursing experience is preferred but not required.
