Description:
The EMPATH Social Worker (MSW) I reports to the Manager of Case Management and Care Transitions. The Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
FLSA: Salaried
Schedule : M-F and Every other weekend
Required License Certification and Registration: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
Minimum Experience and Training Requirements: Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education. MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred. Requires excellent communication skills, attention to detail, and ability to work independently. Demonstrates ability to collaborate in a positive, professional manner with medical professionals. Requires ability to work in a thorough but efficient manner. Experience in an inpatient or outpatient psychiatric setting preferred.
