  • A degree in nursing as a licensed practical nurse.
  • Twelve months of facility or agency nursing experience as a licensed practical nurse is preferred.
  • A valid driver’s license, automobile insurance, and qualification to drive under the organization’s motor vehicle check are required.
  • Good verbal and written communication, comprehension, computer, and interpersonal skills are required.
  • The ability to speak, read, write, and understand English is required.
  • A current Licensed Practical Nurse license in good standing is required in the state or states where the employee practices.
Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
  • Implement the plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Assess patients and provide patient and family or caregiver education and information pertinent to the diagnosis and plan of care.
  • Coordinate home health services and report identified needs for other disciplines to the registered nurse, registered nurse case manager, or Director of Clinical Management.
  • Provide mentoring, coaching, and education to direct care staff under the supervision of the registered nurse.
  • Use equipment and supplies effectively and efficiently.
  • Participate in personal and professional growth and development.
  • Participate in the on-call rotation.
  • Participate in interdisciplinary team meetings to coordinate client and family care, exchange information, solve problems, and receive staff support and education.
  • Maintain a professional manner, manage time efficiently, provide leadership as a role model, collaborate with others, and participate in policy creation and problem-solving.
  • Follow organizational policies, procedures, the Employee Information Guide, the Code of Conduct, facility handbooks, and applicable federal, state, and local regulations and laws.
  • Provide care with respect and dignity, report resident or family complaints to appropriate supervisors, and report allegations of abuse, misappropriation of funds or resident property, and other compliance concerns.
  • Complete annual education requirements, including compliance, privacy, security, and Health Insurance Portability and Accountability Act training.
  • Perform other duties as assigned.
