MDB Health Services brings on-site health care to residents in long-term care and skilled nursing facilities, offering primary care, behavioral and mental health services, wound care, and medical foot care at the resident’s location. A clinician-led team of physicians, nurse practitioners, registered nurses, and licensed clinical social workers provides care within facilities, helping facilities meet CMS standards and avoid external appointments. The service coordinates using PointClickCare and bills patients’ insurance directly, with no cost to partner facilities and acceptance of all insurance types. The company aims to improve resident outcomes by reducing rehospitalizations (reported 10% below the national average) and expand access through a MyTown Health Partners acquisition to serve medically underserved communities.
