It coordinates care for patients through a multi-location footprint, delivering end-of-life and related support to patients at home and in care settings. The company grew primarily by acquiring smaller hospice and home health providers, using a roll-up strategy backed by private equity to quickly expand market share. Its model combines clinical services with a broad geographic network and partnerships with health systems, such as Ascension, to extend care access. The goal is to deliver comprehensive, coordinated palliative and end-of-life care across communities by building a large, integrated network of providers.
