Requirements
  • Physicians must hold an active United States medical license.
  • Physicians must be board-certified in an appropriate specialty.
  • New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
  • Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
  • Candidates must be able to perform the essential functions of the position satisfactorily, with or without reasonable accommodation.
  • Candidates must complete a criminal background check and pre-employment drug testing upon acceptance of a contingent offer.
Responsibilities
  • Serve as the attending physician for patients in each assigned skilled nursing facility.
  • Collaborate with nurse practitioners and physician assistants to manage daily patient care and oversee clinical supervision.
  • Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
  • Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.

Theoria Medical provides medical services for seniors in post-acute and primary care settings, with on-site clinicians, telemedicine, and remote patient monitoring across skilled nursing facilities and other senior living communities. It works along a care continuum from hospital to post-acute care to primary care by offering medical directors, on-site staff, 24/7 telemedicine, and chronic care management to reduce hospital readmissions and improve quality of life. Its in-house software suite includes ChartEasy, an EHR for long-term care; ChatEasy, a secure clinical messaging platform; and Theoria Telemedicine, which integrates with monitoring and analytics to support care coordination and decision-making. The goal is to standardize and improve care across facilities nationwide while optimizing operations, aided by partnerships and growth investments such as Amulet Capital Partners.
