Job Description
Pharmacy Concierge Services is a high touch prescription refill management program that promotes patient-centered continuous care.
- Complete patient outreach medication refill calls.
- Monitor medication adherence and promote compliance.
- Complete patient medication synchronization plans.
- Identify patients at risk of non-adherence and work with a pharmacist to develop strategies to improve compliance.
- Maintain accurate patient records related to medication refills, adherence, and other interventions.
- Evaluate any medication assistance options to promote and optimize guideline driven therapy management.
- Assist in providing chart reviews and medication reconciliation.
- Assist in completing medication prior authorizations.
- Assist in patient assistance programs application process.
- Address any social determinants of health and barriers to the achievement of optimal health goals.
Additional Job Description
Education: High School Degree or Equivalent
Work Experience: 1 year
- Current registration as a Pharmacy Technician by the State of South Carolina Board of Pharmacy (SCBOP), and national certification by the Pharmacy Technician Certification Board (PTCB) is required and must be maintained.
- Participate as an active member of a multidisciplinary healthcare team.
- Work closely with pharmacists, healthcare providers, and other healthcare professionals to ensure comprehensive patient care.
- Utilize and develop strong communication skills to collaborate and disseminate information to patients and other health care providers.
- Enhance professional development and education.
- The program provides a platform to apply knowledge and strengthen skills.
