We are a well-established, multiple provider, family practice currently seeking a FULL-TIME MD or DO Physician to join our team!
Our physicians enjoy practicing population health based medicine across the lifespan in a non-rushed and provider friendly environment. A typical day consists of 16 patients, focusing on delivering comprehensive care in a family centered environment. If you enjoy practicing in a collegial team environment and you want to care for the whole patient, we might be a good fit for you!
We are located in Phoenix, AZ on 19th Ave and Bethany Home Rd. Our practice hours are Monday - Thursday 7:00a.m. - 4:00p.m. and Friday 7:00a.m. - Noon, closed on weekends.

Your primary responsibilities will include conducting the following:
- Well visits to include Well Woman Exams, Complete Annual Physicals, AWV
- Same day sick visits including fever, UTI, headache and allergies
- Chronic disease management including diabetes, hypertension, COPD
- Population Health Management, preventive care, healthy living, disease prevention
- Though we are excited about our recent affiliation with our parent company NextCare, Inc. as we continue to operate solely as a primary care office and see patients by appointment only. You will not be providing urgent care appointments in this position or working at a facility that provides urgent care.
