- Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education
- Licensed as a licensed clinical social worker by the Board of Behavioral Sciences
- Minimum of one (1) year of experience in healthcare, with preference given to candidates with experience in home health care
- Excellent verbal and written communication skills, as well as strong organizational abilities
- Current CPR certification
- Must be a licensed driver with access to a reliable, insured vehicle in good working order
- Assess the psychosocial status of patients in relation to their illness and environment
- Conduct social evaluations and develop intervention plans based on assessment findings
- Maintain accurate clinical records for all patients referred to social work services
- Provide information and referral services to patients and families/caregivers regarding practical and environmental needs
- Serve as a liaison between patients/families/caregivers and community agencies
