Requirements
  • A Bachelor of Science degree and 1 to 2 years of experience, or an equivalent combination of education and experience, is required.
  • Experience working in public health, health care, and/or social work is expected for the ideal candidate.
  • The position requires strong organization and attention to detail.
  • The position requires the ability to work with moderate supervision.
Responsibilities
  • Coordinate day-to-day activities of the Center for Surgical Health program and support operational workflows.
  • Coordinate patients from referral through intake and connection with primary care.
  • Screen patients for program eligibility and enroll eligible patients.
  • Collect and store patient information and related documentation.
  • Organize volunteer Personal Patient Navigator trainings and support volunteer onboarding.
  • Support Personal Patient Navigator-patient dyads throughout the perioperative continuum.
  • Conduct patient intake and enrollment, including screening patients, explaining Center for Surgical Health services to patients and their families, confirming eligibility, and tracking patient progress.
  • Recruit volunteer Personal Patient Navigators through marketing events and support volunteers with navigation steps.
  • Input data into the Center for Surgical Health Salesforce system.
  • Respond to operational queries in a timely manner.
  • Regularly report program progress to managers and upon request.
  • Organize, maintain, and ensure the accuracy of all patient documentation.
