Requirements
  • Graduate from an approved school of nursing.
  • Hold a current Ohio nursing license.
  • Demonstrate flexibility, sensitivity, and adaptability to transplant activities.
  • Demonstrate the knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served.
  • Assess data reflective of the patient's status and formulate an appropriate course of action in collaboration with the transplant team.
Responsibilities
  • Coordinate post-discharge follow-up care for heart and lung transplant recipients after transplantation.
  • Assist with post-discharge coordination of all aspects of post-transplant follow-up care.
  • Act as a liaison among transplant surgeons and physicians, nurses, pharmacies, laboratories, home health agencies, and other support services.
  • Provide patient education as indicated.
Desired Qualifications
  • A Bachelor of Science in Nursing degree is preferred.
  • Prior transplant experience is preferred.
  • Current Certified Clinical Transplant Coordinator certification is preferred.
