Requirements
  • Two to five years of nursing experience with adult and pediatric patients in primary care/ambulatory care, a home health agency, a skilled nursing setting, or a hospital-based medical-surgical setting.
  • A Bachelor degree, specifically a Bachelor of Science in Nursing or Bachelor of Science, is preferred.
  • A current Registered Nurse license in good standing for the State of Michigan is required.
  • Ability to use word-processing, spreadsheet, presentation, database, and other job-relevant software.
  • Ability to work autonomously and remain directly accountable for practices.
  • Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
  • Ability to analyze complex data sets and manage complex clinical issues using assessment skills, resources, and protocols.
  • Knowledge of health concerns and current health-status changes affecting patients from neonates through geriatrics across various medical specialties.
Responsibilities
  • Support ambulatory primary care and specialty practices by triaging calls and coordinating care as needed.
  • Collaborate with primary care practices, specialists, healthcare-team members, patients, and families to support quality, efficient, and cost-effective care.
  • Follow evidence-based clinical guidelines, preventive guidelines, and protocols in patient care.
  • Coordinate care across acute and other ambulatory settings and help patients and families understand healthcare options.
  • Communicate by telephone and in writing with patients, physicians, and other healthcare professionals to exchange pertinent information.
  • Connect patients with Acute Care Managers and other resources as needed.
  • Implement evidence-based interventions and protocols for chronic conditions.
  • Set appropriate boundaries with patients and families and prioritize, delegate, and organize work effectively.
