Responsibilities
  • Coordinate education for staff caring for patients with heart failure and provide community education lectures.
  • Chair the Heart Failure multidisciplinary team.
  • Plan and provide cardiac and pulmonary rehabilitative care to the patient while assuring patient safety as needed.
  • Conduct an individual needs assessment; plan, implement and monitor the patient's exercise prescription and risk-factor reduction plan.
  • Rounding on patients hospitalized with diagnoses of congestive heart failure or potential for congestive heart failure or signs and symptoms thereof.
  • Gather information to ensure that the core measures for quality for this patient diagnosis are achieved.
  • Communicate effectively with physicians involved in the care of these patients.
Desired Qualifications
  • Education from an accredited nursing program
  • Three years of clinical health care or health education experience
  • Current California Registered Nurse license
  • Experience in cardiology nursing
  • Experience with data collection
  • BLS certification
