Requirements
  • A current physical therapist license is required.
  • Knowledge of or experience using Homecare Homebase electronic medical records is a plus.
  • Experience in home health or a related healthcare setting is preferred.
  • Strong clinical and assessment skills are required.
  • Excellent communication and interpersonal abilities are required.
  • Ability to work independently and manage time effectively is required.
Responsibilities
  • Perform ongoing clinical assessments, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and assess patient appropriateness for home care.
  • Develop and evaluate plans of care in partnership with patients, their representatives when applicable, and caregivers.
  • Document, prepare, and complete clinical notes, phone orders, and other clinical record documentation on an ongoing and timely basis, and revise plans of care based on evaluation and ongoing assessment data.
  • Provide physician-ordered services indicated in the plan of care and oversee other team members, when applicable, in delivering patient care consistent with patient needs.
  • Provide supervision and guidance to physical therapy assistants and home health aides, as appropriate.
  • Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and therapy assessments.
  • Communicate with physicians involved in the plan of care and other healthcare practitioners regarding the current plan of care.
  • Meet mandatory continuing education requirements of the agency and licensing board.
  • Assist and instruct patients and families in transfer techniques appropriate to patient abilities.
  • Instruct patients and families on the safe use of equipment such as walkers, wheelchairs, and canes, and use appropriate therapy equipment specific to patients' diagnoses and needs.
  • Provide patient, caregiver, and family counseling and provide patient and caregiver education.
  • Apply infection-control concepts and standard precautions while coordinating or performing patient-care activities to prevent contamination and disease transmission.
  • Accept clinical assignments consistent with education and competence to care for patients.
