- An active Michigan Speech-Language Pathologist license in good standing is required.

- Complete comprehensive in-home Speech-Language Pathologist evaluations and deliver skilled visits.
- Assess and treat aphasia and other communication disorders as ordered or clinically indicated.
- Assess and treat cognitive-communication impairment, including attention, memory, and executive-function issues affecting safety and independence.
- Perform dysphagia screening and assessment and provide swallow-safety education within home-health scope and orders.
- Build and execute a clear plan of care with measurable, functional goals.
- Coordinate multidisciplinary care by communicating changes in condition, aligning on orders, and keeping the care team moving.
- Coach patients and caregivers with practical strategies and routines.
- Document promptly and thoroughly.
- Prior home-health experience.
- High-signal neurological experience with stroke, aphasia, or cognitive impairment.
- Dysphagia-related experience appropriate for home health.
