Requirements
  • Graduate of an accredited school of nursing or have an education equivalency for licensing.
  • Have three years of acute care experience.
  • Hold the required Registered Nurse license for the applicable state of work location: an Illinois Registered Professional Nurse license, a compact-state Registered Nurse license or Missouri Registered Nurse license, a compact-state Registered Nurse license or Oklahoma Registered Nurse license, or a compact-state Registered Nurse license or Wisconsin Registered Nurse license.
  • Be able to frequently lift, carry, push, or pull objects weighing 0–25 pounds.
  • Be able to frequently sit, stand, walk, reach, and perform repetitive foot, leg, hand, and arm movements.
  • Be able to frequently use near and far vision and depth perception to identify and distinguish colors.
  • Be able to frequently use hearing and speech for oral communication and hear alarms and malfunctioning machinery.
  • Be able to frequently use a keyboard and perform data entry.
  • Be able to occasionally bend, stoop, kneel, squat, twist, and grip.
  • Be able to occasionally lift, carry, push, or pull objects weighing 25–50 pounds.
  • Be able to rarely climb.
Responsibilities
  • Ensure compliance with Condition of Participation discharge planning.
  • Identify and escalate gaps and barriers related to inpatient and observation length of stay.
  • Assess patients at high risk for readmission and identify the root cause for readmission.
  • Develop plans and collaborate with the multidisciplinary team regarding discharge planning to the next level of care.
  • Participate with other team members in opportunities to improve standard work.
  • Perform the utilization management process, emergency department case management process, and admission process as assigned.
  • Act as a case manager liaison with post-acute providers, including skilled nursing facilities, rehabilitation facilities, and home health providers, as assigned.
  • Apply evidence-based practice and scientific knowledge in health care to nursing practice, ensuring nursing care is delivered according to patients' age-specific and clinical needs.
  • Follow professional nursing standards defined in the professional practice model and use the American Nurses Association Code of Ethics for Nurses to guide responses to patients' current and evolving health and nursing needs.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
