Requirements
  • Master's or Doctoral Degree from an accredited school of Social Work.
  • Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
  • Minimum of one year experience as a Master of Social Work (MSW) in a healthcare setting.
  • Working knowledge of community resources and good interpersonal skills.
  • Proof of current CPR certification.
  • Reliable transportation with a valid driver's license and auto liability insurance.
Responsibilities
  • Conduct ongoing clinical assessments of home health patients to identify psychosocial, financial, environmental, and community resource needs.
  • Develop and evaluate the plan of care in collaboration with the patient, client, representative (if any), and caregiver(s).
  • Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
  • Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
  • Utilize effective interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
  • Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
  • Participate in the Agency's Quality Assessment and Performance Improvement (QAPI) program and in-service training.
  • Stay updated on public and private eligibility standards and requirements, maintaining professional growth and competency.
  • Promote the Agency's philosophy and administrative policies to ensure the delivery of quality care.
  • Position could see 3-4 visits per month.

Clinicians visit patients at home to deliver services for conditions such as congestive heart failure, orthopedic needs, and infusion nursing, including care for catastrophically injured individuals, while coordinating with families and managing treatment plans. Growth comes from organic expansion, acquisitions of other home health and hospice providers, and building new operations, along with partnerships with referral sources, payors, and providers. Its goal is to deliver high-quality, comfortable care at home and to expand its market reach and service offerings through a network of home health and hospice services.
