Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Tele-Pulmonologist to join our dynamic team. This role will be pivotal in providing advanced respiratory care and leveraging cutting-edge technology to monitor and improve the health outcomes of our long-term care patients.
As a Tele-Pulmonologist with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative remote monitoring solutions. You will be responsible for delivering a comprehensive respiratory therapy program for eligible Skilled Nursing Facility patients with the assistance of an on-site Respiratory Therapist to ensure the highest standards of care.
Key Responsibilities:
- Assessment and Planning: Conduct thorough assessments of patients’ respiratory conditions and develop individualized treatment plans administered by on-site Respiratory Therapists with a wide range of respiratory therapy treatments including aerosol therapies, chest physiotherapy, mechanical ventilation, oxygen therapy, and other advanced modalities as prescribed. Regularly review and adjust plans based on patient progress and changing conditions.
- Patient Monitoring: Assess acute changes in patients’ conditions identified by Circadia Technology and Circadia respiratory therapist through chart review and live A/V tele-assessment.
- Interdisciplinary Team Coordination: Work collaboratively with Respiratory Therapists, Skilled Nursing physicians, nurses, and other healthcare professionals to ensure a comprehensive approach to patient care.
- Documentation: Maintain and review detailed, accurate, and timely documentation of patient assessments, treatments, progress notes, and any changes in condition. Ensure all records are compliant with facility policies and regulatory standards.
Requirements:
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school with a Pulmonology speciality.
- Board-certified and licensed to practice medicine in the state of residence.
- Previous experience in telemedicine, remote monitoring, or medical oversight is preferred.
- Familiarity with remote patient monitoring technologies and EHR systems.
- Strong attention to detail and ability to interpret medical data.
- Excellent communication and interpersonal skills for remote collaboration with a diverse team.
- Empathy and a patient-centered approach to provide compassionate remote care.
- Ability to adapt to new technologies and work effectively in a remote environment.
